
Navigating Complex Care: Webinar recording
Between 2015 and 2050, the proportion of the world’s population over 60 years will nearly double from 12% to 22%. The number of persons aged 80 years or older is expected to triple between 2020 and 2050 to reach 426 million.* Developing systemic approaches to supporting our aging populations is top of mind of health systems in the United States and across the globe.
In this Kaiser Permanente International webinar, leaders from our Northern California market share two initiatives that are providing holistic, person-centered care, designed to help older adults age in place.
Melissa Stern, Regional Director of Strategy and Program Operations for Complex Needs, Geriatrics, and Continuing Care, The Permanente Medical Group, shared Kaiser Permanente Northern California’s approach to caring for older adults with complex care needs, highlighting how we leverage data, multi-disciplinary teams, and holistic support to personalize care for our patients. Heather Brown, Executive Director, Continuum Care Coordination and Maite Echaniz, Senior Operations Director, Continuum Care, shared how Kaiser Permanente is collaborating with Habitat Health to bring the evidence-based Program of All-Inclusive Care for the Elderly to more older adults in communities served by Kaiser Permanente.
