Kaiser Permanente’s integrated model leverages primary care to as a central place for our members to receive preventative and chronic condition management. We do this by using the evidence-based practices that support patient-centered care, delivered by a team of interdisciplinary health professionals. Katie Coleman leads the ACT (Accelerating Care Transformation) Center in our Washington region. She will share the evidence behind our primary care approach and the many resources available to health professionals looking to implement similar systems.

Empowering Patients through Health Education

Health education is a core part of Kaiser Permanente’s integrated care delivery model, and one that augments our primary care. Through health coaching, online and in-person classes, health education works with clients across demographics to provide culturally competent education and skills to support total health. This session will share how integral health education is to meeting patients where they are at and providing resources and tools to help members and patients manage their health and wellness.
