About the program
Kaiser Permanente created Preventing Heart Attacks and Strokes Everyday (PHASE), a comprehensive population management program to control cardiovascular risks for diabetic patients.
Since its inception, PHASE has continually shown success in managing blood pressure, lipids, and blood sugar l evels. The creator of PHASE, Marc Jaffe, MD, will share his experience and lessons learned when implementing this approach.

Marc is the guideline director for the Kaiser Permanente Northern California Region and chief of endocrinology at the Kaiser Permanente San Francisco Medical Center. He joined Kaiser Permanente in 1993 and led the design of the Kaiser Permanente Northern California Hypertension Program in 2000. In 2004 he led the creation of the Prevent Heart Attacks and Strokes Everyday (PHASE) program, which delivers cardiovascular preventive services to high-risk individuals. Dr. Jaffe served as the senior vice president of the Resolve to Save Lives Cardiovascular Health Initiative, a program with Vital Strategies, from 2017 to 2019.
